Program Terms:

  • Membership is valid for 12 months starting on the date of enrollment.
  • The total fee is due at the time of purchase; installment payments are not available.
  • Memberships are non-refundable and may not be transferred to another person. Benefits cannot be refunded or credited if they are not used.
  • This membership is not dental insurance and cannot be combined with any dental insurance benefits, discount programs, or promotional offers.

ADULT PLAN
